You can update details like your name, address, phone number or email address:
- Through the Anima service
- By calling us on 01233 624642 or visit the surgery from 10am to 6pm
Changing your name
You will need to show us proof that you have changed your name. This can be one of the following:
- Marriage Certificate
- Civil Partnership Certificate
- Deed Poll
- Decree Absolute (Final Order) (where reverting to a previous name and supported by appropriate documentation
Changing your phone number or email address
You may be required to show us identification before we can change your phone number or email address, this can be in the form of photographic ID. We may also request you to verify your new details to evidence you have access to the new number or email.
Change of address
If you have moved to a new address, you will need to be in our catchment area to stay registered with our surgery. You may be required to show us proof of your new address. If you are required, this can be one of the following:
- Council Tax Bill (current year)
- Utility Bill (dated within the last 3 months)
- Bank or Building Society Statement (dated within the last 3 months)
- HMRC Correspondence (dated within the last 3 months)
- Tenancy Agreement
- Mortgage Statement
